Authorize your Repairs

I hereby authorize Dreamworks Collision, Center, Inc. (hereafter referred to as DCC) to conduct the repair of my vehicle.  I understand that I will be informed by DCC of any deficiencies in the estimate that will prevent the vehicle from being restored to its pre-accident condition, to the best of reasonable human ability and be given notice and the opportunity to authorize additional repairs or parts replacement as needed to fully and properly repair my vehicle.  I further understand additional costs may be incurred. It is the repair facility's responsibility to do those repairs for which they receive correct compensation in a professional, high-quality manner with any and all modifications provided in writing in the final billing.  Furthermore, I agree that I will fully and unconditionally hold harmless the DCC, its employees, directors, affiliates, owners, managers, and their heirs from all actions, legal and otherwise, known or unknown, and from all expenses incurred to defend such actions, including reasonable attorney fees, from any entity, insurance company, or third party that may elect to take such action associated with the repair and or removal of this automobile.

It is further understood and agreed that the contract of repair hereby entered into by the undersigned with DCC may not be assigned or transferred to any other parties or persons without the specific written consent of DCC.

I have been advised by this document that all charges must be paid in full before the vehicle can be released.  Procuring payment from another party, if applicable, will be the sole responsibility of the vehicle owner.  However, DCC will assist whenever possible to expedite the process.  Failure to take delivery of the vehicle more than 3-days after the repair has been completed may cause additional charges and storage to accrue at prescribed charges.  In the event repairs are halted or terminated, prior to completion, storage charges, stall tie up, administration charges, and other incidental charges may be assessed from the date my vehicle arrives to such time as it is removed from DCC.
